SeniorBridge Hospitalization Risk Assessment Tool: Managing Complex Chronic Care (MC3™)

Many older adults are at risk of being hospitalized but few want to be admitted if they can help it.

MC3™ is a free assessment tool developed by Eric C. Rackow, MD, president and CEO of SeniorBridge and immediate past president of NYU Hospital, along with his team of renowned clinicians to help assess a seniors risk of being hospitalized. The tool is intended to empower families and individuals with awareness and to aid in the creation of a plan for staying healthy at home.
